Is Your Data Telling the Full Story?

Community Action Agencies change lives every day by helping individuals and families achieve greater stability and self-sufficiency. This self-assessment helps leadership teams evaluate whether their current data practices provide a complete picture of services delivered, outcomes achieved, and opportunities for Continuous Quality Improvement (CQI).

How to Use This Assessment

Review each question with your leadership team. The goal is not simply to identify gaps, but to identify opportunities to strengthen processes, improve collaboration across programs, and ensure your data reflects the full impact of your agency.

Rating Scale

☐ Consistently (2 points)
We have a reliable process in place, information is captured consistently, and leadership can easily access this information when needed.

☐ Sometimes (1 point)
We complete this activity, but processes may vary by staff member, location, or department. Information may require additional effort to locate or analyze.

☐ Needs Improvement (0 points)
We rely on manual processes, information is not consistently captured, or leadership does not have clear visibility into this area.

How to Calculate Your Score

  1. Review each statement in the assessment.

  2. Select one response for each statement.

  3. Assign points for each response:

    1. Consistently = 2 points

    2. Sometimes = 1 point

    3. Needs Improvement = 0 points

  4. Add your points together for a maximum possible score of 80 points.
