Head Start Data Visibility Self-Assessment

Is Your Data Telling the Full Story?

Programs do incredible work every day to support children, families, and communities. This self-assessment helps leadership teams evaluate whether their current data practices provide a complete picture of program activities, outcomes, and opportunities for continuous quality improvement.

How to Use This Assessment

Review each question with your leadership team. The goal is not simply to identify gaps, but to identify opportunities to strengthen processes and ensure your data reflects the impact of your work.

Rating Scale

☐ Consistently (2 points)
We have a reliable process in place, information is captured consistently, and leadership can easily access this information when needed.

☐ Sometimes (1 point)
We complete this activity, but processes may vary by staff member, location, or department. Information may require additional effort to locate or analyze.

☐ Needs Improvement (0 points)
We rely on manual processes, information is not consistently captured, or leadership does not have clear visibility into this area.

Understanding Your Data Visibility Maturity Level

Score Range

Data Visibility Level

What This Means

0–23 Points

Building the Foundation

Your program is collecting valuable information, but there are opportunities to improve consistency, ownership, and visibility.

24-46 Points

Improving Visibility

Your program has established processes, but may benefit from stronger reporting, analysis, and connections between daily work and program outcomes.

47-70 Points

Data-Driven Program Management

Your program is using data to identify trends, make informed decisions, and support continuous quality improvement.

Regardless of your current level, every program has opportunities to strengthen how information is captured, reviewed, and used. The goal is not more data. The goal is meaningful data that helps programs tell the complete story of the work they do every day.
